CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. This sequence change replaces lysine with asparagine at codon 68 of the CFTR protein (p.Lys68Asn). The lysine residue is weakly conserved and there is a moderate physicochemical difference between lysine and asparagine. This variant is present in population databases (rs397508335, ExAC 0.003%). This missense change has been observed in individual(s) with chronic pancreatitis and cystic fibrosis (PMID: 12452372, 28040058). ClinVar contains an entry for this variant (Variation ID: 53431). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.K68N variant (also known as c.204A>T), located in coding exon 3 of the CFTR gene, results from an A to T substitution at nucleotide position 204. The lysine at codon 68 is replaced by asparagine, an amino acid with similar properties. This alteration has been identified in multiple individuals with pancreatitis (Gaia E et al. Dig Dis Sci, 2002 Nov;47:2416-21; Sofia VM et al. Mol Med, 2016 Sep;22:300-309). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: CFTR c.204A>T (p.Lys68Asn) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 251032 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.204A>T has been reported in the literature as a non-informative genotype (second allele of uncertain significance or not reported) in individuals affected with Pancreatitis, Cystic Fibrosis or in settings for newborn screening (example, Sofia_2016, Gaia_2002, Handschick_2012, Bozdogan_2020). These report(s) do not provide unequivocal conclusions about association of the variant with Cystic Fibrosis. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 33572515, 12452372, 21917531, 24777605, 25735457, 27264265). Five submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
