The p.S56R variant (also known as c.168C>A), located in coding exon 2 of the CDKN2A gene, results from a C to A substitution at nucleotide position 168. The serine at codon 56 is replaced by arginine, an amino acid with dissimilar properties. This variant was detected in a cohort of 587 patients with either multiple or single primary melanomas who were sequenced for CDKN2A alterations (Bruno W et al. J Am Acad Dermatol, 2016 Feb;74:325-32). This variant was also detected in a cohort of 85 male breast cancer patients (Al Saati A et al. Int J Mol Sci, 2023 Sep;24). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

The CDKN2A gene encodes two different proteins, p16INK4a and p14ARF, which are translated from alternative transcripts with different open reading frames. Both transcripts have been analyzed. We report either the variant with the higher classification or default to the CDKN2A (p16INK4a) variant. This report therefore includes the details for the CDKN2A (p16INK4a) variant. This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 56 of the CDKN2A (p16INK4a) protein (p.Ser56Arg). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with multiple primary melanomas (PMID: 2677576). This variant is also known as c.211C>A (p.Arg71Ser) in the CDKN2A (p14ARF) transcript. ClinVar contains an entry for this variant (Variation ID: 236981). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. This variant disrupts the p.Ser56 amino acid residue in CDKN2A (p16INK4a). Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 9425228, 9516223, 12072543, 15150307, 15235029, 17492760, 20340136, 21462282, 22841127). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
