Variant summary
Our verdict is Uncertain significance. Variant got 4 ACMG points: 4P and 0B. PM2PP3PP5
The NM_000551.4(VHL):c.462A>G(p.Pro154=) variant causes a splice region, synonymous change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000137 in 1,461,352 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. P154P) has been classified as Pathogenic.

Likely pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Mar 25, 2020 | Not observed at significant frequency in large population cohorts (gnomAD); Has not been previously published as pathogenic or benign to our knowledge; In silico analysis supports a deleterious effect on splicing; This variant is associated with the following publications: (PMID: 26920352, 21463266, 17006605, 9829912) - |

Von Hippel-Lindau syndrome;C1837915:Chuvash polycythemia Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Nov 07, 2018 | This variant has not been reported in the literature in individuals with VHL-related disease.  ClinVar contains an entry for this variant (Variation ID: 411977). This variant is not present in population databases (ExAC no frequency). This sequence change affects codon 154 of the VHL mRNA. It is a 'silent' change, meaning that it does not change the encoded amino acid sequence of the VHL protein. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. A different variant (c.462A>C) affecting this nucleotide with the same silent change has been reported to segregate with disease in families affected with von Hippel-Lindau syndrome (PMID: 9829912, 17006605), and experimental studies suggest that this nucleotide may be crucial for normal mRNA splicing (PMID: 17006605). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site, but this prediction has not been confirmed by published transcriptional studies. - |
