Variant summary
Our verdict is Likely pathogenic. Variant got 9 ACMG points: 9P and 0B. PM2PM5PP3_StrongPP5
The NM_004863.4(SPTLC2):āc.529A>Gā(p.Asn177Asp) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000000684 in 1,461,878 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N177H) has been classified as Likely pathogenic.

Neuropathy, hereditary sensory and autonomic, type 1C Pathogenic:2
For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects SPTLC2 function (PMID: 30955194). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt SPTLC2 protein function. ClinVar contains an entry for this variant (Variation ID: 430350). This missense change has been observed in individuals with clinical features of hereditary sensory and autonomic neuropathy (PMID: 30955194; Invitae). It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces asparagine, which is neutral and polar, with aspartic acid, which is acidic and polar, at codon 177 of the SPTLC2 protein (p.Asn177Asp). -

not provided Uncertain:1
A variant of uncertain significance has been identified in the SPTLC2 gene. The N177D variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. However, a different missense variant in the same position (N177H) has been reported previously in an individual with hereditary sensory neuropathy (Antoniadi et al., 2015). The N177D variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The N177D variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. This substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant. -
