Genes affected
FH (HGNC:3700): (fumarate hydratase) The protein encoded by this gene is an enzymatic component of the tricarboxylic acid (TCA) cycle, or Krebs cycle, and catalyzes the formation of L-malate from fumarate. It exists in both a cytosolic form and an N-terminal extended form, differing only in the translation start site used. The N-terminal extended form is targeted to the mitochondrion, where the removal of the extension generates the same form as in the cytoplasm. It is similar to some thermostable class II fumarases and functions as a homotetramer. Mutations in this gene can cause fumarase deficiency and lead to progressive encephalopathy. Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Aug 12, 2022 | This missense change has been observed in individuals with hereditary leiomyomatosis and renal cell cancer (Invitae). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces glycine, which is neutral and non-polar, with glutamic acid, which is acidic and polar, at codon 146 of the FH protein (p.Gly146Glu). ClinVar contains an entry for this variant (Variation ID: 186519). For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FH protein function. - |

Hereditary cancer-predisposing syndrome Pathogenic:1
Likely pathogenic, criteria provided, single submitter | clinical testing | Ambry Genetics | Aug 14, 2023 | The p.G146E variant (also known as c.437G>A), located in coding exon 4 of the FH gene, results from a G to A substitution at nucleotide position 437. The glycine at codon 146 is replaced by glutamic acid, an amino acid with a few similar properties. This alteration has been identified in an individual meeting clinical diagnostic criteria for HLRCC (Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on internal structural analysis, this variant is likely to have a destabilizing effect on, and thus affect the substrate binding site (Ambry internal data; Phan I et al. Acta Crystallogr. Sect. F Struct. Biol. Cryst, 2011 Sep;67:1123-8; Weaver T et al. J. Mol. Biol., 1998 Jul;280:431-42). Based on the majority of available evidence to date, this variant is likely to be pathogenic. - |
