Variant summary
Our verdict is Pathogenic. Variant got 13 ACMG points: 13P and 0B. PM2PM5PP3PP5_Very_Strong
The NM_000784.4(CYP27A1):c.1183C>A(p.Arg395Ser) variant causes a missense, splice region change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000958 in 1,461,856 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 2/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R395C) has been classified as Pathogenic.

Pathogenic, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Jan 15, 2024 | Variant summary: CYP27A1 c.1183C>A (p.Arg395Ser) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. Consensus agreement among computation tools predict no significant impact on normal splicing. However, one publication reports experimental evidence that this variant affects mRNA splicing and is expected to lead to the loss of protein expression (Chen_1998). The variant allele was found at a frequency of 8e-06 in 251236 control chromosomes. c.1183C>A has been reported in the literature in individuals affected with Cerebrotendinous Xanthomatosis and clinical frontotemporal dementia (Chen_1998, Blauwendraat_2018). Additionally, the demonstrated alternative pre-mRNA splicing was shown to lead to decreased sterol 27-hydroxylase activity (Chen_1998). Other variants at the same codon have been widely reported to be pathogenic (p.Arg395Cys, p.Arg395His). ClinVar contains an entry for this variant (Variation ID: 4261). Based on the evidence outlined above, the variant was classified as pathogenic. - |

Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Oct 14, 2023 | This sequence change replaces arginine, which is basic and polar, with serine, which is neutral and polar, at codon 395 of the CYP27A1 protein (p.Arg395Ser). This variant is present in population databases (rs121908096, gnomAD 0.002%). This missense change has been observed in individual(s) with clinical features of cerebrotendinous xanthomatosis (PMID: 9790667, 28749476). This variant is also known as p.Arg362Ser. ClinVar contains an entry for this variant (Variation ID: 4261). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Studies have shown that this missense change alters mRNA splicing and is expected to lead to the loss of protein expression (PMID: 9790667). This variant disrupts the p.Arg395 amino acid residue in CYP27A1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 2019602, 20402754, 26156051). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. - |
