Variant summary
Our verdict is Likely pathogenic. The variant received 6 ACMG points: 6P and 0B. PM1PM2PP3_Moderate
The NM_000400.4(ERCC2):c.2137G>C(p.Gly713Arg) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000112 in 1,613,954 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G713A) has been classified as Uncertain significance.

Variant summary: ERCC2 c.2137G>C (p.Gly713Arg) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251302 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2137G>C has been reported in the literature as a compound heterozygous genotype in at-least one individual affected with Trichothiodystrophy who has been subsequently cited by others (example, Takayama_1996, Taylor_1997, Botta_1998, Lanzafame_2022). These data do not allow any conclusion about variant significance. At least one publication reports experimental evidence evaluating an impact on protein function reporting abolished interaction with the p44 subunit of TFIIH resulting in reduced helicase activity of XPD (Dubaele_2003). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance. -
