This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. This sequence change falls in intron 57 of the ATM gene. It does not directly change the encoded amino acid sequence of the ATM protein. RNA analysis indicates that this variant induces altered splicing and likely results in a shortened protein product. This variant is present in population databases (no rsID available, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with ATM-related conditions. ClinVar contains an entry for this variant (Variation ID: 479066). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in skipping of 57, but is expected to preserve the integrity of the reading-frame (PMID: 32133419). This variant disrupts the c.8418+5G nucleotide in the ATM gene. Other variant(s) that disrupt this nucleotide have been determined to be pathogenic (PMID: 8808599; internal data). This suggests that this nucleotide is clinically significant, and that variants that disrupt this position are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

Variant summary: ATM c.8418+5G>A alters a conserved nucleotide located close to a canonical splice site and therefore could affect mRNA splicing, leading to a significantly altered protein sequence. Several computational tools predict a significant impact on normal splicing: Two predict the variant abolishes a canonical 5' splicing donor site and two predict the variant weakens this site. At least one publication reports experimental evidence that this variant affects mRNA splicing, resulting in an in-frame exon skipping (Landrith_2020). The variant allele was found at a frequency of 8e-06 in 251006 control chromosomes (gnomAD). c.8418+5G>A has been reported in the literature in an individual affected with Breast Cancer (Landrith_2020). The following publication has been ascertained in the context of this evaluation (PMID: 32133419). ClinVar contains an entry for this variant (Variation ID: 479066). The c.8418+5G>A intronic variant results from a G to A substitution 5 nucleotides after coding exon 56 in the ATM gene. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. RNA studies showed abnormal splicing in the set of samples tested (Ambry internal data). Another alteration impacting the same donor site, c.8418+5_8418+8delGTGA, has been shown to have a similar impact on splicing and has been reported in multiple individuals with a clinical diagnosis of ataxia-telangiectasia (A-T) (Wright J et al. Am. J. Hum. Genet. 1996 Oct;59(4):839-46; Stankovic T et al. Am. J. Hum. Genet. 1998 Feb;62(2):334-45; Li A and Swift M. Am. J. Med. Genet. 2000 May;92(3):170-7; Buzin CH et al. Hum. Mutat. 2003 Feb;21(2):123-31). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -
