Genes affected
RYR1 (HGNC:10483): (ryanodine receptor 1) This gene encodes a ryanodine receptor found in skeletal muscle. The encoded protein functions as a calcium release channel in the sarcoplasmic reticulum but also serves to connect the sarcoplasmic reticulum and transverse tubule. Mutations in this gene are associated with malignant hyperthermia susceptibility, central core disease, and minicore myopathy with external ophthalmoplegia. Alternatively spliced transcripts encoding different isoforms have been described. [provided by RefSeq, Jul 2008]

RYR1-related disorder Uncertain:2
Uncertain significance, no assertion criteria providedclinical testingPreventionGenetics, part of Exact SciencesJul 01, 2024The RYR1 c.7082C>G variant is predicted to result in the amino acid substitution p.Pro2361Arg. This variant is also predicted to activate a cryptic donor site within the exon (SpliceAI, Jaganathan K, et al. 2019. PubMed ID: 30661751). This variant has been reported to occur de novo in an individual with a developmental disorder; however, detailed phenotypic information was not available and the patient also had a de novo variant in TREX1 (Kaplanis et al. 2020. PubMed ID: 33057194; Zhou et al. 2022. PubMed ID: 35982159). To our knowledge, this variant has not been reported a large population database, indicating this variant is rare. At PreventionGenetics, we have observed this variant on the same allele with another RYR1 variant in a patient with malignant hyperthermia, ptosis, and scoliosis (Internal Data). This variant is located in exon 44 which is a hotspot region for variants that are associated with malignant hyperthermia susceptibility. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
Uncertain significance, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpJan 05, 2017Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). This sequence change is located in the central mutational hotspot of the RYR1 protein where a significant number of previously reported RYR1 missense mutations are found (PMID: 16084090). In summary, this variant is a novel missense change with uncertain impact on protein function. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may alter RNA splicing, but this prediction has not been confirmed by published transcriptional studies. This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a RYR1-related disease. This sequence change replaces proline with arginine at codon 2361 of the RYR1 protein (p.Pro2361Arg). The proline residue is highly conserved and there is a moderate physicochemical difference between proline and arginine. -
