Variant summary
Our verdict is Pathogenic. The variant received 17 ACMG points: 17P and 0B. PM1PM2PM5PP2PP3_ModeratePP5_Very_Strong
The NM_000044.6(AR):c.2324G>A(p.Arg775His) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 12/20 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R775C) has been classified as Pathogenic.

Androgen resistance syndrome;C1839259:Kennedy disease Pathogenic:1
This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 775 of the AR protein (p.Arg775His). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with androgen insensitivity syndrome (PMID: 1307250, 1609793, 8723113, 10690872, 20150575, 24737579, 25613104). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 9819). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on AR protein function. Experimental studies have shown that this missense change affects AR function (PMID: 1307250, 1480178, 1609793). This variant disrupts the p.Arg775 amino acid residue in AR. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 1307250, 1609793, 8723113, 20150575). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
