This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 102 of the PDE6A protein (p.Arg102Cys). This variant is present in population databases (rs141252097, gnomAD 0.006%). This missense change has been observed in individual(s) with clinical features of autosomal recessive retinitis pigmentosa (PMID: 18723146; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 839256). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PDE6A protein function with a positive predictive value of 80%. This variant disrupts the p.Arg102 amino acid residue in PDE6A. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 10393062, 25775262, 27917291). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -

The p.Arg102Cys variant in PDE6A was identified in an individual with Retinitis pigmentosa, via a collaborative study between the Broad Institute's Center for Mendelian Genomics and the Pierce lab (https://oculargenomics.meei.harvard.edu/labs/pierce-lab/lab-members/). Through a review of available evidence we were able to apply the following criteria: PS1, PM2, PP1, PP3. Based on this evidence we have classified this variant as Likely Pathogenic.
