The encoded protein can bind NF-kappa-B-inducing kinase and I-kappa-B kinases through separate domains and assemble them into an active kinase complex. However, this alteration occurs at the 3' terminus and the exact functional impact is unknown at this time. This nucleotide position is highly conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to abolish the native splice donor site; however, direct evidence is unavailable. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Several computational tools predict a significant impact on normal splicing: Four predict the variant abolishes a 5 splicing donor site. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 5.3e-05 in 282618 control chromosomes (gnomAD). This frequency is not significantly higher than expected for a pathogenic variant in IKBKAP causing Familial Dysautonomia (5.3e-05 vs 0.0018), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.3931+1G>T in individuals affected with Familial Dysautonomia and no experimental evidence demonstrating its impact on protein function have been reported. Two ClinVar submitters (evaluation after 2014) cite this variant as likely pathogenic and uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
