This missense variant replaces arginine with glutamine at codon 2414 of the FBN1 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in two related individuals with Marfan-related phenotype from a family with history of Marfan syndrome (PMID: 16835936). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.R2414Q variant (also known as c.7241G>A), located in coding exon 58 of the FBN1 gene, results from a G to A substitution at nucleotide position 7241. The arginine at codon 2414 is replaced by glutamine, an amino acid with highly similar properties. This alteration has been reported in a Marfan syndrome cohort, noting individuals with this alteration did not fulfill Ghent criteria (Sakai H et al. Am J Med Genet A, 2006 Aug;140:1719-25). Additionally, this alteration was detected in an ectopia lentis cohort in an individual with an additional alteration in FBN1 identified (Chen Z et al. Br J Ophthalmol, 2022 Dec;106:1655-1661). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
