Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with malignant hyperthermia susceptibility (PMID: 31559918). This variant has been identified in 15/282054 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 3853 of the RYR1 protein (p.Glu3853Lys). This variant is present in population databases (rs145087576, gnomAD 0.02%). This missense change has been observed in individual(s) with autosomal recessive RYR1-related conditions (PMID: 22526018, 30611313). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant has been reported in individual(s) with autosomal dominant malignant hyperthermia (PMID: 31559918); however, the role of the variant in this condition is currently unclear. ClinVar contains an entry for this variant (Variation ID: 161376). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt RYR1 protein function with a positive predictive value of 80%. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 5.2e-05 in 250704 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in RYR1 causing Myopathy, RYR1-Associated, allowing no conclusion about variant significance. c.11557G>A has been reported in the literature in compound heterozygous individuals affected with Myopathy, RYR1-Associated (Vasli_2012, Garibaldi_2019). c.11557G>A has also been reported in a heterozygous individual with autosomal dominant malignant hyperthermia (Sadhasivam_2019). These data indicates that this variant may be associated with diseases. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
