EYA4 (HGNC:3522): (EYA transcriptional coactivator and phosphatase 4) This gene encodes a member of the eyes absent (EYA) family of proteins. The encoded protein may act as a transcriptional activator through its protein phosphatase activity, and it may be important for eye development, and for continued function of the mature organ of Corti. Mutations in this gene are associated with postlingual, progressive, autosomal dominant hearing loss at the deafness, autosomal dominant non-syndromic sensorineural 10 locus. The encoded protein is also a putative oncogene that mediates DNA repair, apoptosis, and innate immunity following DNA damage, cellular damage, and viral attack. Defects in this gene are also associated with dilated cardiomyopathy 1J. Alternative splicing results in multiple transcript variants encoding distinct isoforms. [provided by RefSeq, Jul 2014]

The Val432Leu variant in EYA4 has not been reported in individuals with hearing loss but has been identified in 0.02% (1/4406) of African American chromosomes b y the NHLBI Exome Sequencing Project (http://evs.gs.washington.edu/EVS; rs145253 833). Although this variant has been seen in the general population, its frequen cy is not high enough to rule out a pathogenic role. Computational analyses (bio chemical amino acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) do not provide strong support for or against an impact to the protein. In summary, additional information is needed to determine the clinical significance of this variant. -

This sequence change replaces valine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 432 of the EYA4 protein (p.Val432Leu). This variant is present in population databases (rs145253833, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with EYA4-related conditions. ClinVar contains an entry for this variant (Variation ID: 163452). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt EYA4 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.V432L variant (also known as c.1294G>C), located in coding exon 14 of the EYA4 gene, results from a G to C substitution at nucleotide position 1294. The valine at codon 432 is replaced by leucine, an amino acid with highly similar properties. This alteration was noted in one individual from a cohort ascertained from a biobank; clinical information was limited (Ahmadmehrabi S et al. Hum Genet, 2021 Jun;140:957-967). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
