MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

The MLH1 p.Leu642* variant was identified in 2 of 164 proband chromosomes (frequency: 0.01) from individuals or families with colorectal cancer (Guindalini 2015, Plimmer 2012). The variant was also identified in ClinVar (classified as pathogenic by Ambry Genetics). The variant was not identified in dbSNP, COGR, Cosmic, UMD-LSDB, Zhejiang University Database, Mismatch Repair Genes Variant Database, the Insight Hereditary Tumors database, the 1000 Genomes Project, the NHLBI GO Exome Sequencing Project, the Exome Aggregation Consortium (August 8th 2016), or the Genome Aggregation Database (Feb 27, 2017). The c.1923delT variant leads to a premature stop codon at position 642, which is predicted to lead to a truncated or absent protein and loss of function. Loss of function variants of the MLH1 gene are an established mechanism of disease in Lynch syndrome and this is the type of variant expected to cause the disorder. In summary, based on the above information this variant meets our laboratory’s criteria to be classified as pathogenic. -

The c.1923delT pathogenic mutation, located in coding exon 17 of the MLH1 gene, results from a deletion of one nucleotide at nucleotide position 1923, causing a translational frameshift with a predicted alternate stop codon. A study of 25 Jamaican early-onset (<40 years old) colorectal cancer patients identified this mutation in one patient with a MSI-H tumor (Plummer JM et al. Can J Surg, 2012 Oct;55:294-300). This mutation was also found in 1/57 African American probands with colorectal cancer in a retrospective study assessing the colorectal cancer risk and mutation spectrum in African American families with Lynch syndrome (Guindalini RS et al. Gastroenterology, 2015 Nov;149:1446-53). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). As such, this alteration is interpreted as a disease-causing mutation. -
