Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_000546.6(TP53):c.499C>T(p.Gln167*) variant causes a stop gained change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. Q167Q) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

This variant is considered pathogenic. This variant creates a termination codon and is predicted to result in premature protein truncation. -

not provided Pathogenic:3
Nonsense variant predicted to result in protein truncation or nonsense mediated decay in a gene for which loss-of-function is a known mechanism of disease; Not observed at a significant frequency in large population cohorts (Lek 2016); This variant is associated with the following publications: (PMID: 18555592, 10519380, 29979965, 11051239, 30886117, 12067251, 22768918) -

Hereditary cancer-predisposing syndrome Pathogenic:3
ACMG/AMP, PVS1, PM2, PP5 -
The p.Q167* pathogenic mutation (also known as c.499C>T), located in coding exon 4 of the TP53 gene, results from a C to T substitution at nucleotide position 499. This changes the amino acid from a glutamine to a stop codon within coding exon 4. This alteration was identified as a somatic alteration in AML and MDS tumors, and found to have between 0-10% of wildtype transactivation activity across multiple response elements tested (Lodé L et al. Haematologica, 2018 Jan;103:e13-e16). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

Li-Fraumeni syndrome Pathogenic:1
ClinVar contains an entry for this variant (Variation ID: 528250). This sequence change creates a premature translational stop signal (p.Gln167*) in the TP53 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in TP53 are known to be pathogenic (PMID: 20522432). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with gastric carcinoma or myelodysplastic syndromes (PMID: 21080251, 27741277). For these reasons, this variant has been classified as Pathogenic. -
