TNNI3 (HGNC:11947): (troponin I3, cardiac type) Troponin I (TnI), along with troponin T (TnT) and troponin C (TnC), is one of 3 subunits that form the troponin complex of the thin filaments of striated muscle. TnI is the inhibitory subunit; blocking actin-myosin interactions and thereby mediating striated muscle relaxation. The TnI subfamily contains three genes: TnI-skeletal-fast-twitch, TnI-skeletal-slow-twitch, and TnI-cardiac. This gene encodes the TnI-cardiac protein and is exclusively expressed in cardiac muscle tissues. Mutations in this gene cause familial hypertrophic cardiomyopathy type 7 (CMH7) and familial restrictive cardiomyopathy (RCM). Troponin I is useful in making a diagnosis of heart failure, and of ischemic heart disease. An elevated level of troponin is also now used as indicator of acute myocardial injury in patients hospitalized with moderate/severe Coronavirus Disease 2019 (COVID-19). Such elevation has also been associated with higher risk of mortality in cardiovascular disease patients hospitalized due to COVID-19. [provided by RefSeq, Aug 2020]

The Thr119Ile variant in the TNNI3 gene has not been reported as a disease-causing mutation or as a benign polymorphism to our knowledge. Thr119Ile results in a semi-conservative amino acid substitution of a neutral, polar Threonine to a non-polar Isoleucine at a position that is moderately conserved across species. In silico analysis predicts Thr119Ile is benign to the protein structure/function. However, nearby mutations (Ala116Gly, Asp127Tyr) have been reported in association with cardiomyopathy, supporting the functional significance of this region of the protein. In addition, the NHLBI ESP Exome Variant Server reports Thr119Ile was not observed in approximately 6000 samples from individuals of European and African American backgrounds, indicating it is not a common benign variant in these populations.The variant is found in DCM panel(s). -

This sequence change replaces threonine, which is neutral and polar, with isoleucine, which is neutral and non-polar, at codon 119 of the TNNI3 protein (p.Thr119Ile). This variant is present in population databases (rs184709702, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with TNNI3-related conditions. ClinVar contains an entry for this variant (Variation ID: 181579). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The isoleucine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.T119I variant (also known as c.356C>T), located in coding exon 6 of the TNNI3 gene, results from a C to T substitution at nucleotide position 356. The threonine at codon 119 is replaced by isoleucine, an amino acid with similar properties. This amino acid position is conserved. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
