Genes affected
SLC6A5 (HGNC:11051): (solute carrier family 6 member 5) This gene encodes a sodium- and chloride-dependent glycine neurotransmitter transporter. This integral membrane glycoprotein is responsible for the clearance of extracellular glycine during glycine-mediated neurotransmission. This protein is found in glycinergic axons and maintains a high presynaptic pool of neurotransmitter at glycinergic synapses. Mutations in this gene cause hyperekplexia; a heterogenous neurological disorder characterized by exaggerated startle responses and neonatal apnea. Two transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Jan 2016]

Pathogenic, criteria provided, single submitterclinical testingInvitaeFeb 08, 2022This sequence change replaces alanine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 228 of the SLC6A5 protein (p.Ala228Asp). This variant is present in population databases (rs371265931, gnomAD 0.002%). For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SLC6A5 protein function. ClinVar contains an entry for this variant (Variation ID: 540364). This missense change has been observed in individual(s) with hyperekplexia (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. -

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsJun 24, 2022The c.683C>A (p.A228D) alteration is located in exon 4 (coding exon 4) of the SLC6A5 gene. This alteration results from a C to A substitution at nucleotide position 683, causing the alanine (A) at amino acid position 228 to be replaced by an aspartic acid (D). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
