Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 0P and 2B. BP4_Moderate
The NM_001036.6(RYR3):c.11945G>A(p.Arg3982Gln) variant causes a missense change. The variant allele was found at a frequency of 0.0000514 in 1,613,844 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R3982W) has been classified as Uncertain significance.

Submissions by phenotype
not specified Uncertain:1
The c.11945G>A (p.R3982Q) alteration is located in exon 89 (coding exon 89) of the RYR3 gene. This alteration results from a G to A substitution at nucleotide position 11945, causing the arginine (R) at amino acid position 3982 to be replaced by a glutamine (Q). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
Epileptic encephalopathy Uncertain:1
This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 3982 of the RYR3 protein (p.Arg3982Gln). This variant is present in population databases (rs374921732, gnomAD 0.02%). This variant has not been reported in the literature in individuals affected with RYR3-related conditions. ClinVar contains an entry for this variant (Variation ID: 530970). Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0". The glutamine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
