The p.R375W variant (also known as c.1123C>T), located in coding exon 6 of the DES gene, results from a C to T substitution at nucleotide position 1123. The arginine at codon 375 is replaced by tryptophan, an amino acid with dissimilar properties. This variant was detected in a cardiomyopathy genetic testing cohort; however, clinical details were limited, and additional cardiac variants were detected in some cases (van Lint FHM et al. Neth Heart J, 2019 Jun;27:304-309). Additionally, this alteration was reported in a sudden infant death cohort (Neubauer J et al. Eur J Hum Genet, 2017 Apr;25:404-409). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
