The HBB c.-51T>C variant has been reported in the published literature in an infant with sickle cell disease who also carried the pathogenic Hb S variant (PMID: 26658910 (2016)). The frequency of this variant in the general population, 0.0019 (46/24808 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Based on the available information, we are unable to determine the clinical significance of this variant. -

Variant summary: HBB c.-51T>C is located in the untranscribed region upstream of the HBB gene region. Consensus agreement among computation tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 0.00017 in 280600 control chromosomes, predominantly at a frequency of 0.0019 within the African or African-American subpopulation in the gnomAD database. This frequency is not significantly higher than estimated for a pathogenic variant in HBB causing Hemoglobinopathy (0.00017 vs 0.011), allowing no conclusion about variant significance. c.-51T>C has been observed in individual(s) affected with Hemoglobinopathy (Kunz_2016, Poon_2021). These report(s) do not provide unequivocal conclusions about association of the variant with Hemoglobinopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
