Variant summary: CLN5 c.433C>T (p.Arg145X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 2.4e-05 in 251424 control chromosomes. c.433C>T has been reported in the literature in individuals affected with Neuronal Ceroid-Lipofuscinosis (Batten Disease)(example: Kousi_2012, Giannakis_2016, Dong_2020). These data indicate that the variant is likely to be associated with disease. One publication reports experimental evidence evaluating an impact of a proline substitution in the same position having a significant impact on protein function, where the authors postulate that this region may be a mutational hotspot where changes are associated with high potential for pathogenicity (Luo_2020). Three ClinVar submitters have assessed this variant after 2014: one have classified the variant as likely pathogenic and two as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
