Variant summary

Our verdict is Likely pathogenic. Variant got 9 ACMG points: 9P and 0B. PM1PM2PM5PP2PP3_Moderate

The NM_000138.5(FBN1):c.7184G>T(p.Gly2395Val) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G2395R) has been classified as Likely pathogenic.

Genes affected
FBN1 (HGNC:3603): (fibrillin 1) This gene encodes a member of the fibrillin family of proteins. The encoded preproprotein is proteolytically processed to generate two proteins including the extracellular matrix component fibrillin-1 and the protein hormone asprosin. Fibrillin-1 is an extracellular matrix glycoprotein that serves as a structural component of calcium-binding microfibrils. These microfibrils provide force-bearing structural support in elastic and nonelastic connective tissue throughout the body. Asprosin, secreted by white adipose tissue, has been shown to regulate glucose homeostasis. Mutations in this gene are associated with Marfan syndrome and the related MASS phenotype, as well as ectopia lentis syndrome, Weill-Marchesani syndrome, Shprintzen-Goldberg syndrome and neonatal progeroid syndrome. Uncertain significance, criteria provided, single submitterclinical testingLaboratory for Molecular Medicine, Mass General Brigham Personalized MedicineAug 14, 2012Variant classified as Uncertain Significance - Favor Pathogenic. The Gly2395Val variant in FBN1 has not been reported in the literature nor previously identifie d by our laboratory. Computational analyses (biochemical amino acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) suggest that the Gly2395Val varian t may impact the protein, though this information is not predictive enough to de termine pathogenicity. In summary, additional information is needed to fully ass ess the clinical significance of the Gly2395Val variant. -

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsJul 06, 2017The p.G2395V variant (also known as c.7184G>T), located in coding exon 57 of the FBN1 gene, results from a G to T substitution at nucleotide position 7184. The glycine at codon 2395 is replaced by valine, an amino acid with dissimilar properties, and is located in the TGFBP #07 domain. This alteration has been reported in an individual with some Marfan-like features; however, clinical details were limited (Lerner-Ellis JP et al. Mol. Genet. Metab., 2014 Jun;112:171-6). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
Marfan syndrome;C4707243:Familial thoracic aortic aneurysm and aortic dissection Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingInvitaeJan 08, 2021In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). This variant has been observed in individual(s) with clinical features of FBN1-related disease (PMID: 24793577, Invitae). ClinVar contains an entry for this variant (Variation ID: 42423). This variant is not present in population databases (ExAC no frequency). This sequence change replaces glycine with valine at codon 2395 of the FBN1 protein (p.Gly2395Val). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and valine. -
