Genes affected
CEP290 (HGNC:29021): (centrosomal protein 290) This gene encodes a protein with 13 putative coiled-coil domains, a region with homology to SMC chromosome segregation ATPases, six KID motifs, three tropomyosin homology domains and an ATP/GTP binding site motif A. The protein is localized to the centrosome and cilia and has sites for N-glycosylation, tyrosine sulfation, phosphorylation, N-myristoylation, and amidation. Mutations in this gene have been associated with Joubert syndrome and nephronophthisis and the presence of antibodies against this protein is associated with several forms of cancer. ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Loss of function variant, product undergoes nonsense mediated mRNA decay. LoF is a known mechanism of disease.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 12-88129717-C-A is Pathogenic according to our data. Variant chr12-88129717-C-A is described in ClinVar as [Likely_pathogenic]. Clinvar id is 813995.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Leber congenital amaurosis 10 Pathogenic:1
Likely pathogenic, criteria provided, single submitterresearchBroad Center for Mendelian Genomics, Broad Institute of MIT and HarvardDec 03, 2018The heterozygous p.Glu277Ter variant in CEP290 was identified by our study in the compound heterozygous state, with a VUS, in one individual with Leber congenital amaurosis. The p.Glu277Ter variant in CEP290 has not been previously reported in individuals with Leber congenital amaurosis and was absent from large population studies. This nonsense variant leads to a premature termination codon at position 277, which is predicted to lead to a truncated or absent protein. Loss of function of the CEP290 gene is an established disease mechanism in autosomal recessive Leber congenital amaurosis. In summary, although additional studies are required to fully establish its clinical significance, the clinial significance of this variant is likely pathogenic. Pathogenic, criteria provided, single submitterclinical testingInvitaeSep 15, 2021For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 813995). This variant has not been reported in the literature in individuals affected with CEP290-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Glu277*) in the CEP290 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in CEP290 are known to be pathogenic (PMID: 16909394, 17345604, 20690115). -
