The p.D857H variant (also known as c.2569G>C), located in coding exon 21 of the ACTN2 gene, results from a G to C substitution at nucleotide position 2569. The aspartic acid at codon 857 is replaced by histidine, an amino acid with similar properties. This variant was described in an individual with sporadic hypertrophic cardiomyopathy (HCM), who harbored an additional cardiac variant (Xu J et al. Sci Rep, 2015 Nov;5:16609). This variant was also detected in a cardiomyopathy genetic testing cohort; however, clinical details were limited (van Lint FHM et al. Neth Heart J, 2019 Jun;27:304-309). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
