Congenital contractural arachnodactyly Uncertain:2
This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 1021 of the FBN2 protein (p.Arg1021Cys). This variant is present in population databases (rs541842635, gnomAD 0.009%). This missense change has been observed in individual(s) with scoliosis (PMID: 24833718). ClinVar contains an entry for this variant (Variation ID: 213297). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt FBN2 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). Publications were found based on this search. However, the evidence from the literature, in combination with allele frequency data from public databases where available, was not sufficient to rule this variant in or out of causing disease. Therefore, this variant is classified as a variant of unknown significance. -
Familial thoracic aortic aneurysm and aortic dissection Uncertain:1
The p.R1021C variant (also known as c.3061C>T), located in coding exon 24 of the FBN2 gene, results from a C to T substitution at nucleotide position 3061. The arginine at codon 1021 is replaced by cysteine, an amino acid with highly dissimilar properties. This variant was reported in one individual from an adolescent idiopathic scoliosis cohort; however, clinical details were limited (Buchan JG et al. Hum Mol Genet, 2014 Oct;23:5271-82). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: FBN2 c.3061C>T (p.Arg1021Cys) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.2e-05 in 1613984 control chromosomes. The observed variant frequency is approximately 34 fold of the estimated maximal expected allele frequency for a pathogenic variant in FBN2 causing Aortopathy phenotype (1.3e-06). c.3061C>T has been reported in the literature in at-least one individual affected with adolescent idiopathic scoliosis (Buchan_2014). These report(s) do not provide unequivocal conclusions about association of the variant with Aortopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 24833718). ClinVar contains an entry for this variant (Variation ID: 213297). Based on the evidence outlined above, the variant was classified as likely benign. -
