Genes affected
KIF1A (HGNC:888): (kinesin family member 1A) The protein encoded by this gene is a member of the kinesin family and functions as an anterograde motor protein that transports membranous organelles along axonal microtubules. Mutations at this locus have been associated with spastic paraplegia-30 and hereditary sensory neuropathy IIC. Alternatively spliced transcript variants encoding distinct isoforms have been described. [provided by RefSeq, Apr 2012]

This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 380 of the KIF1A protein (p.Arg380Trp). This variant is present in population databases (rs545501989, gnomAD 0.002%). This missense change has been observed in individual(s) with clinical features of autosomal dominant hereditary spastic paraplegia (PMID: 31805580). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 583174). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt KIF1A protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -

Significance:Uncertain significance
Review Status:criteria provided, single submitter
Collection Method:clinical testing

The p.R380W variant (also known as c.1138C>T), located in coding exon 12 of the KIF1A gene, results from a C to T substitution at nucleotide position 1138. The arginine at codon 380 is replaced by tryptophan, an amino acid with dissimilar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Significance:Uncertain significance
Review Status:criteria provided, single submitter
Collection Method:curation

This variant is interpreted as a variant of uncertain significance for NESCAV syndrome, autosomal dominant. The following ACMG Tag(s) were applied: Multiple lines of computational evidence support a deleterious effect on the gene or gene product (PP3); Missense variant in a gene that has a low rate of benign missense variation and in which missense variants are a common mechanism of disease (PP2); Assumed de novo, but no confirmation of paternity and maternity (PM6). -
