The variant was identified in dbSNP (ID: rs587780041) as “With Uncertain significance allele”, ClinVar (as uncertain significance by GeneDx and Invitae, and as likely benign by Ambry Genetics), and Clinvitae (3x) databases. The variant was identified in control databases in 2 of 30932 chromosomes at a frequency of 0.000065 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: African in 1 of 8716 chromosomes (freq: 0.000115), European (Non-Finnish) in 1 of 14990 chromosomes (freq: 0.000067); it was not observed in the Ashkenazi Jewish, East Asian, European (Finnish), Latino, Other, and South Asian populations. The p.Lys413 residue is not conserved in mammals and the variant amino acid Arginine (Arg) is present in many animals, including rats, mice, and dogs, increasing the likelihood that this variant does not have clinical significance. The variant was identified with a co-occurring pathogenic STK11 variant (p.Asp194Asn), increasing the likelihood that the p.Lys413Arg variant does not have clinical significance. Computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; however, this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time although we would lean towards a more benign role for this variant. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
