For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt TGFBR2 protein function. ClinVar contains an entry for this variant (Variation ID: 165391). This missense change has been observed in individual(s) with Loeys-Dietz syndrome (PMID: 24146167). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 287 of the TGFBR2 protein (p.Trp287Arg). -
Loeys-Dietz syndrome Pathogenic:1
The p.Trp287Arg variant in TGFBR2 has been identified in 1 individual with Loeys-Dietz syndrome as a de novo variant, though maternity and paternity were not confirmed (LMM Data, Ting 2014). This variant was absent from large population data. Computational prediction tools and conservation analyses suggest an impact to the protein. In summary, although additional studies are required to fully establish its clinical significance, this variant meets criteria to be classified as likely pathogenic for autosomal dominant Loeys-Dietz syndrome.
