Variants in RYR1 can also cause other myopathies inherited in an autosomal dominant pattern or in an autosomal recessive pattern. Some of these disorders may predispose individuals to malignant hyperthermia. RYR1 variants may also contribute to a malignant hyperthermia reaction in combination with other genetic and non-genetic factors and the clinician needs to consider such factors in making management decisions.This sequence variant predicts a substitution of valine with methionine at codon 2354 of the RYR1 protein, p.(Val2354Met). This variant was not present in a large population database (gnomAD) at the time this variant was interpreted. This variant has been reported in two unrelated individuals who have a personal or family history of a malignant hyperthermia reaction; both of these individuals had a positive in vitro contracture test (IVCT) or caffeine halothane contracture test (CHCT) result (if the proband was unavailable for testing, a positive diagnostic test result in a mutation-positive relative was counted), PS4_Moderate (PMID:23558838, PMID:24361844). This variant segregates with MHS in five individuals PP1_Moderate (PMID:24361844). An ex vivo functional study on lymphoblasts from a single family was published for this variant showing a decreased EC50 for 4-CmC, multiple unrelated samples are required to fulfil PS3 (PMID:2461844). This variant resides in a region of RYR1 considered to be a hotspot for pathogenic variants that contribute to MHS, PM1 (PMID:21118704). A REVEL score >0.85 (0.867) supports a pathogenic status for this variant, PP3_Moderate. This variant has been classified as Likely Pathogenic. In addition, in vitro measurement of contracture response in an individual harboring this RYR1 variant who exhibited masseter muscle rigidity was abnormal for both caffeine and halothane, consistent with malignant hyperthermia susceptibility (PMID: 30864471). This missense variant is located in an RYR1 hotspot where most mutations linked to MH are clustered (PMID: 16084090). This variant is not present in populations databases (gnomAD) and in-silico predictions suggest it is deleterious. Therefore the c.7060G>A (p.Val2354Met) RYR1 variant is considered likely pathogenic. - |
