Variant summary
Our verdict is Uncertain significance. The variant received 3 ACMG points: 4P and 1B. PM2PP3_ModerateBS2_Supporting
The NM_000465.4(BARD1):c.1886G>T(p.Trp629Leu) variant causes a missense change. The variant allele was found at a frequency of 0.00000342 in 1,461,584 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W629R) has been classified as Uncertain significance.

Familial cancer of breast Uncertain:2
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. ClinVar contains an entry for this variant (Variation ID: 460729). This missense change has been observed in individual(s) with male breast cancer (PMID: 30613976). This variant is present in population databases (rs747446711, gnomAD 0.0009%). This sequence change replaces tryptophan, which is neutral and slightly polar, with leucine, which is neutral and non-polar, at codon 629 of the BARD1 protein (p.Trp629Leu). -

Hereditary cancer-predisposing syndrome Uncertain:2
The p.W629L variant (also known as c.1886G>T), located in coding exon 9 of the BARD1 gene, results from a G to T substitution at nucleotide position 1886. The tryptophan at codon 629 is replaced by leucine, an amino acid with similar properties. This variant was identified in 1 of 523 BRCA1/2 negative male breast cancer patients undergoing multigene panel testing (Rizzolo P et al. Int J Cancer, 2019 Jul;145:390-400). This variant was also detected in a cohort of 1/1663 Brazilian breast cancer patients who underwent hereditary multigene panel testing (Guindalini RSC et al. Sci Rep, 2022 Mar;12:4190). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
This missense variant replaces tryptophan with leucine at codon 629 of the BARD1 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with male breast cancer (PMID: 30613976). This variant has been identified in 1/251198 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
