This sequence change replaces methionine with isoleucine at codon 470 of the BMPR1A protein (p.Met470Ile). The methionine residue is highly conserved and there is a small physicochemical difference between methionine and isoleucine. This variant is present in population databases (rs749743579, ExAC 0.001%). This variant has not been reported in the literature in individuals with BMPR1A-related conditions. ClinVar contains an entry for this variant (Variation ID: 485366). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
This missense variant replaces methionine with isoleucine at codon 470 of the BMPR1A protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with BMPR1A-related disorders in the literature. This variant has been identified in 1/251482 chromosomes in the general population by the Genome Aggregation Database (gnomAD). Different variants affecting the same codon, c.1409T>G (p.Met470Arg) and c.1409T>C (p.Met470Thr), are considered to be disease-causing (ClinVar variation ID: 1771973, 8236), suggesting that this position may be important for the protein function. The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.M470I variant (also known as c.1410G>A), located in coding exon 10 of the BMPR1A gene, results from a G to A substitution at nucleotide position 1410. The methionine at codon 470 is replaced by isoleucine, an amino acid with highly similar properties. While this exact alteration has not been reported in the literature, an alteration at the same codon, p.M470T (c.1409T>C) has been reported in a Korean juvenile polyposis (JP) patient and was not seen in 96 unrelated healthy individuals (Kim IJ et al, Clin. Genet. 2003 Feb; 63(2):126-30). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6500 samples (13000 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 65000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.M470I remains unclear. -
