BMPR1A (HGNC:1076): (bone morphogenetic protein receptor type 1A) The bone morphogenetic protein (BMP) receptors are a family of transmembrane serine/threonine kinases that include the type I receptors BMPR1A and BMPR1B and the type II receptor BMPR2. These receptors are also closely related to the activin receptors, ACVR1 and ACVR2. The ligands of these receptors are members of the TGF-beta superfamily. TGF-betas and activins transduce their signals through the formation of heteromeric complexes with 2 different types of serine (threonine) kinase receptors: type I receptors of about 50-55 kD and type II receptors of about 70-80 kD. Type II receptors bind ligands in the absence of type I receptors, but they require their respective type I receptors for signaling, whereas type I receptors require their respective type II receptors for ligand binding. The BMPR1A p.Gly251Cys variant was not identified in the literature nor was it identified in the Cosmic, MutDB, or LOVD 3.0, databases. The variant was identified in dbSNP (ID: rs750513716) as "With Uncertain significance allele ", and in ClinVar database (classified as uncertain significance by Invitae, Ambry Genetics, Color Genomics and Integrated Genetics/Laboratory Corporation of America). The variant was identified in control databases in 8 of 246178 chromosomes at a frequency of 0.00003 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: Latino in 3 of 33572 chromosomes (freq: 0.00009), European in 5 of 111668 chromosomes (freq: 0.00005), while the variant was not observed in the African, Other, Ashkenazi Jewish, East Asian, Finnish, and South Asian populations. The p.Gly251 residue is conserved in mammals but not in more distantly related organisms, and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. Variant summary: The BMPR1A c.751G>T (p.Gly251Cys) variant causes a missense change involving a conserved nucleotide with 5/5 in silico tools predicting a damaging outcome, although these predictions have yet to be functionally assessed. The variant of interest was observed in the large, broad control population, ExAC with an allele frequency of 6/120956 (1/20161), which does exceed the estimated maximal expected allele frequency for a pathogenic BMPR1A variant of 1/500000 (0.000002). However, this observation needs to be cautiously considered because the database does indicate low coverage at the cite, in addition, the cohort could harbor individuals that have a BMPR1A phenotype. The variant of interest, to our knowledge, has not been reported in affected individuals via publications and/or reputable databases/clinical laboratories. Therefore, taking all available lines of evidence into consideration, the variant of interest has been classified as a "VUS-possibly benign," until additional information becomes available.

This sequence change replaces glycine, which is neutral and non-polar, with cysteine, which is neutral and slightly polar, at codon 251 of the BMPR1A protein (p.Gly251Cys). This variant is present in population databases (rs750513716, gnomAD 0.009%). This variant has not been reported in the literature in individuals affected with BMPR1A-related conditions. ClinVar contains an entry for this variant (Variation ID: 460511). Invitae Evidence Modeling incorporating data from in vitro experimental studies (internal data) indicates that this missense variant is not expected to disrupt BMPR1A function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population.
