Variant summary
Our verdict is Likely pathogenic. The variant received 8 ACMG points: 8P and 0B. PM1PM2PP3_Strong
The NM_000135.4(FANCA):c.3761A>T(p.Glu1254Val) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000124 in 1,613,710 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. E1254Q) has been classified as Uncertain significance.

Inborn genetic diseases Uncertain:1
The c.3761A>T (p.E1254V) alteration is located in exon 37 (coding exon 37) of the FANCA gene. This alteration results from a A to T substitution at nucleotide position 3761, causing the glutamic acid (E) at amino acid position 1254 to be replaced by a valine (V). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
Fanconi anemia Uncertain:1
This sequence change replaces glutamic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 1254 of the FANCA protein (p.Glu1254Val). This variant is present in population databases (rs750773229, gnomAD 0.005%). This variant has not been reported in the literature in individuals affected with FANCA-related conditions. ClinVar contains an entry for this variant (Variation ID: 237051). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on FANCA protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
