Variant summary
Our verdict is Benign. The variant received -7 ACMG points: 2P and 9B. PM1BP6BS1BS2
The NM_177438.3(DICER1):āc.2390A>Gā(p.Asp797Gly) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000204 in 1,614,024 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D797N) has been classified as Uncertain significance.

DICER1-related disorder Uncertain:1
The DICER1 c.2390A>G variant is predicted to result in the amino acid substitution p.Asp797Gly. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.023% of alleles in individuals of South Asian descent in gnomAD, which is more common than expected for an undocumented cause of disease. In ClinVar, this variant has conflicting interpretations including likely benign and uncertain significance (https://www.ncbi.nlm.nih.gov/clinvar/variation/477102/). Although we suspect that this variant may be benign, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -

Hereditary cancer-predisposing syndrome Uncertain:1
The p.D797G variant (also known as c.2390A>G), located in coding exon 14 of the DICER1 gene, results from an A to G substitution at nucleotide position 2390. The aspartic acid at codon 797 is replaced by glycine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
