Variant summary
Our verdict is Likely pathogenic. Variant got 8 ACMG points: 8P and 0B. PM1PM2PP3_Strong
The NM_003000.3(SDHB):c.425A>T(p.Asp142Val) variant causes a missense, splice region change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 2/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D142G) has been classified as Uncertain significance.

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. ClinVar contains an entry for this variant (Variation ID: 565490). This missense change has been observed in individual(s) with thyroid cancer (PMID: 25694510). This variant is present in population databases (rs759709073, gnomAD 0.0009%). This sequence change replaces aspartic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 142 of the SDHB protein (p.Asp142Val). -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.D142V variant (also known as c.425A>T), located in coding exon 5 of the SDHB gene, results from an A to T substitution at nucleotide position 425. The aspartic acid at codon 142 is replaced by valine, an amino acid with highly dissimilar properties. This variant has been reported in an individual with thyroid cancer (Ni Y et al. Endocr Relat Cancer, 2015 Apr;22:121-30). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
