Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 5e-05 in 1,606,776 control chromosomes (i.e. in 80 carriers) in the gnomAD database v4.0 dataset. This frequency is not higher than the estimated maximum expected for a pathogenic variant in MYH7 causing Cardiomyopathy (0.0013), allowing no conclusion about variant significance. However, the presence of the variant in several carriers makes it unlikely that it is associated with a dominant, early onset, high penetrance, severe disease phenotype. c.5471A>G has been reported in the literature in an individual affected with hypertrophic cardiomyopathy (HCM), and in an individual with long QT syndrome (LQTS) (Walsh_2017, Robyns_2017). These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The asparagine at codon 1824 is replaced by serine, an amino acid with highly similar properties. This variant was detected in a cohort referred for hypertrophic cardiomyopathy genetic testing (Walsh R et al. Genet. Med., 2017 02;19:192-203). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis.
