Genes affected
TSC1 (HGNC:12362): (TSC complex subunit 1) This gene is a tumor suppressor gene that encodes the growth inhibitory protein hamartin. The encoded protein interacts with and stabilizes the GTPase activating protein tuberin. This hamartin-tuberin complex negatively regulates mammalian target of rapamycin complex 1 (mTORC1) signaling which is a major regulator of anabolic cell growth. This protein also functions as a co-chaperone for Hsp90 that inhibits its ATPase activity. This protein functions as a facilitator of Hsp90-mediated folding of kinase and non-kinase clients, including TSC2 and thereby preventing their ubiquitination and proteasomal degradation. Mutations in this gene have been associated with tuberous sclerosis and lymphangioleiomyomatosis. [provided by RefSeq, May 2022]

This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). Publications were found based on this search. The evidence from the literature, in combination with allele frequency data from public databases where available, was sufficient to rule this variant out of causing disease. Therefore, this variant is classified as benign. -

Variant summary: TSC1 c.1250C>T (p.Thr417Ile) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.00012 in 1607130 control chromosomes, predominantly at a frequency of 0.0042 within the East Asian subpopulation in the gnomAD database. The observed variant frequency within East Asian control individuals in the gnomAD database is approximately 170-fold of the estimated maximal expected allele frequency for a pathogenic variant in TSC1 causing Tuberous Sclerosis Complex phenotype (2.5e-05). In addition, the variant was reported in some East Asian subpopulations with an even higher allele frequency, e.g. in the Japanese, with an allele frequency of 0.0082 (i.e. 989 / 119880 alleles), including 4 homozygotes (in the jMorp database; PMID: 33179747), suggesting that the variant is a benign polymorphism. This variant has been ascertained in HGMD, however comments suggest non-supporting evidence for pathogenic role. Publications also reported experimental evidence evaluating an impact on protein function and demonstrated no damaging effect of this variant (e.g. Pymar_2008). The following publication have been ascertained in the context of this evaluation (PMID: 18397877). ClinVar contains an entry for this variant (Variation ID: 48752). Based on the evidence outlined above, the variant was classified as benign. -

Variant identified in a genome or exome case(s) and assessed due to predicted null impact of the variant or pathogenic assertions in the literature or databases. Disclaimer: This variant has not undergone full assessment. The following are preliminary notes: This variant has been reported in 4 papers in HGMD but comments suggest not pathogenic as it has been seen in unaffected individuals and there was no LOH in tumors of carriers. It has a Max MAF of 0.05% in ExAC (4 alleles) and 0.03% in gnomAD (6 alleles). The variant is present in ClinVar but with no classification. The variant is predicted to be benign by prediction tools. This AA is not conserved and 3 mammals have an Ile at this position. -

This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). No publications were found based on this search. Allele frequency data from public databases was too high to be consistent with this variant causing disease. Therefore, this variant is classified as benign. -
