Variant summary: RYR1 c.4657G>A (p.Val1553Ile) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be tolerated. The variant allele was found at a frequency of 4.4e-05 in 251442 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in RYR1 causing Congenital multicore myopathy with external ophthalmoplegia (4.4e-05 vs 0.0011), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.4657G>A in individuals affected with Congenital multicore myopathy with external ophthalmoplegia and no experimental evidence demonstrating its impact on protein function have been reported.
