The encoded protein functions as a calcium release channel in the sarcoplasmic reticulum but also serves to connect the sarcoplasmic reticulum and transverse tubule. Mutations in this gene are associated with malignant hyperthermia susceptibility, central core disease, and minicore myopathy with external ophthalmoplegia. This missense variant replaces asparagine with lysine at codon 1346 of the RYR1 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with congenital myopathy, who carried a different pathogenic variant in the same gene (PMID: 29382405). This variant has not been reported in an individual with malignant hyperthermia susceptibility in the literature. This variant has been identified in 17/185820 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Variant summary: RYR1 c.4038C>A (p.Asn1346Lys) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 9.7e-05 in 154564 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in RYR1 causing Congenital multicore myopathy with external ophthalmoplegia (9.7e-05 vs 0.0011), allowing no conclusion about variant significance. c.4038C>A has been reported in the literature in individuals affected with Congenital multicore myopathy with external ophthalmoplegia (Wu_2018, Clayton_2024). These report(s) do not provide unequivocal conclusions about association of the variant with Congenital multicore myopathy with external ophthalmoplegia. Co-occurrences with other pathogenic variant(s) have been reported (RYR1 c.13220delG, p.Gly4407Valfs*34), providing supporting evidence for a benign role.
