The variant received 9 classification points (ACMG Germline Pathogenicity v2019). PS3PM1PM2_SupportingPP3_Moderate

The NM_000059.4(BRCA2):c.7961T>C (p.Leu2654Pro) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a cumulative frequency of 0.00000186 (AC=3) in the gnomAD database across 1,614,066 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.0000299. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. The affected nucleotide is highly conserved across species (PhyloP 100-way vertebrate score: 7.93). Variant has been reported in ClinVar as Uncertain Significance (★★★). ClinVar reports functional evidence for this variant: "SCV000666056: This variant is non-functional in a cell-based homology-directed DNA break repair (HDR) functional assay (Guidugli L et al. Cancer Res. 2013 Jan;73(1):265-75" and additional evidence is available in ClinVar. Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.L2654= (synonymous): Likely_benign (ClinVar VariationId 4215194, 1 star); p.L2654= (synonymous): Likely_benign (ClinVar VariationId 4215398, 1 star); p.L2654del: Uncertain_significance (ClinVar VariationId 827362, 2 stars)

Genes affected
BRCA2 (HGNC:1101): (BRCA2 DNA repair associated) Inherited mutations in BRCA1 and this gene, BRCA2, confer increased lifetime risk of developing breast or ovarian cancer. Both BRCA1 and BRCA2 are involved in maintenance of genome stability, specifically the homologous recombination pathway for double-strand DNA repair. The largest exon in both genes is exon 11, which harbors the most important and frequent mutations in breast cancer patients. The BRCA2 gene was found on chromosome 13q12.3 in human. The BRCA2 protein contains several copies of a 70 aa motif called the BRC motif, and these motifs mediate binding to the RAD51 recombinase which functions in DNA repair. BRCA2 is considered a tumor suppressor gene, as tumors with BRCA2 mutations generally exhibit loss of heterozygosity (LOH) of the wild-type allele. [provided by RefSeq, May 2020]

Classification according to ACMG Germline Pathogenicity v2019

Classification was made for transcript

Our verdict: Likely_pathogenic. The variant received 9 points.

PS3
Well-established functional study supports damaging effect (PS3); SCV000666056: This variant is non-functional in a cell-based homology-directed DNA break repair (HDR) functional assay (Guidugli L et al. Cancer Res. 2013 Jan;73(1):265-75; Hart SN et al. Genet Med, 2019 01;21:71-80; Hu C et al. Am J Hum Genet. 2024 Mar;111(3):584-593). Two saturation genome editing-based studies, including a haploid cell-survival assay and a humanized mouse embryonic stem cell line assay of drug response and survival, demonstrate that this nucleotide substitution is non-functional(Huang H et al. Nature. 2025 Feb;638(8050):528-537; Sahu S et al. Nature. 2025 Feb;638(8050):538-545).
PM1
Missense neighbourhood hotspot (≥2 P/LP within ±8 AA, OR ≥ 5 vs. background) — PM1; In-domain: 0 pathogenic, 0 benign rare missense variants.; ±8 AA neighbourhood: 14 pathogenic, 6 benign (OR vs. background: 11.2).
PM2
Rare in gnomAD for AD/XL gene (popmax AF below threshold) — PM2 Supporting; GnomAD popmax AF = 0.0000299 — borderline rare for AD+AR gene (base 0.0001, raised to 0.000139 by highest known P/LP ClinVar AF 0.000139) — PM2 supporting.
PP3
Germline meta computational scorer (REVEL/MetaRNN/BayesDel) predicts damaging effect — moderate evidence (PP3); Splicing verdict: not pathogenic.; Germline computational verdict: pathogenic (Moderate).
