BRCA1 (HGNC:1100): (BRCA1 DNA repair associated) This gene encodes a 190 kD nuclear phosphoprotein that plays a role in maintaining genomic stability, and it also acts as a tumor suppressor. The BRCA1 gene contains 22 exons spanning about 110 kb of DNA. The encoded protein combines with other tumor suppressors, DNA damage sensors, and signal transducers to form a large multi-subunit protein complex known as the BRCA1-associated genome surveillance complex (BASC). This gene product associates with RNA polymerase II, and through the C-terminal domain, also interacts with histone deacetylase complexes. This protein thus plays a role in transcription, DNA repair of double-stranded breaks, and recombination. Mutations in this gene are responsible for approximately 40% of inherited breast cancers and more than 80% of inherited breast and ovarian cancers. Alternative splicing plays a role in modulating the subcellular localization and physiological function of this gene. Many alternatively spliced transcript variants, some of which are disease-associated mutations, have been described for this gene, but the full-length natures of only some of these variants has been described. It was also identified in this individual's affected sister, who was diagnosed with breast cancer in her early 40s. To our knowledge, there are no reports of this variant in the literature in association with disease. This variant is absent in large population databases including the Genome Aggregation Database. This variant is located within the BRCT domain and is predicted to impact protein function (BayeDel >=0.28). It has been reported by one calibrated study to affect protein function similar to pathogenic control variants (Findlay et al. 2018). Using the ClinGen ENIGMA BRCA1 Expert Panel Specifications to the ACMG/AMP Variant Interpretation Guidelines for BRCA1 Version 1.1.0, this variant was classified as Likely Pathogenic (PS3_Strong, PM2_Supporting, PP3_Supporting, PP4_Supporting). -

To our knowledge, no occurrence of c.5107T>G in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome has been reported. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in loss of homology directed repair (HDR) activity in a high throughput assay measuring cellular fitness in a haploid cell line whose survival is dependent on intact BRCA1 function (Findlay_2018). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014.
