In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant is found within a region of MYH7 between codons 181 and 937 that contains the majority of the myosin head domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with hypertrophic cardiomyopathy (PMID: 27532257). Experimental studies have shown that this variant affects MYH7 protein function (PMID: 31213605). This variant has been observed in individual(s) with hypertrophic cardiomyopathy (PMID: 20031618, Invitae). It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (ExAC no frequency). This sequence change replaces aspartic acid with tyrosine at codon 382 of the MYH7 protein (p.Asp382Tyr). The aspartic acid residue is highly conserved and there is a large physicochemical difference between aspartic acid and tyrosine. -

The p.D382Y variant (also known as c.1144G>T), located in coding exon 11 of the MYH7 gene, results from a G to T substitution at nucleotide position 1144. The aspartic acid at codon 382 is replaced by tyrosine, an amino acid with highly dissimilar properties. This variant has been reported in subjects with hypertrophic cardiomyopathy (HCM) (Lopes LR et al. Heart, 2015 Feb;101:294-301; Kaski JP et al. Circ Cardiovasc Genet, 2009 Oct;2:436-41: Ambry internal data; external communication). This alteration is located in the myosin head domain, which contains a statistically significant clustering of pathogenic missense variants (Homburger JR et al. Proc Natl Acad Sci U S A, 2016 06;113:6701-6; Walsh R et al. Genet Med, 2017 02;19:192-203; Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
