Variant summary
Our verdict is Likely pathogenic. Variant got 7 ACMG points: 7P and 0B. PM1PM2PP2PP3PP5
The NM_014874.4(MFN2):c.526G>A(p.Gly176Ser) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000000684 in 1,461,858 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars).

Charcot-Marie-Tooth disease type 2A2 Pathogenic:1
Likely pathogenic based on conservation and prediction scores (Phylop, LRT, MutationTaster). Identified in homozygous state in individual with peripheral axonal neuropathy; onset at 18 months. Parents are carriers but unaffected clinically. -
Charcot-Marie-Tooth disease, axonal, autosomal recessive, type 2a2b; Pathogenic:1

Charcot-Marie-Tooth disease type 2 Uncertain:1
This sequence change replaces glycine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 176 of the MFN2 protein (p.Gly176Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with Charcot-Marie-Tooth disease (PMID: 26257172, 29361379). ClinVar contains an entry for this variant (Variation ID: 243074). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on MFN2 protein function. This variant disrupts the p.Gly176 amino acid residue in MFN2. Other variant(s) that disrupt this residue have been observed in individuals with MFN2-related conditions (PMID: 28286897), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
