The c.1434del p.(Leu479TrpfsTer28) variant in LDLR is a frameshift variant predicted to create a premature stop codon leading to nonsense mediated decay in a gene in which loss-of-function is an established disease mechanism (PVS1). This variant is absent from gnomAD v4.1.0 (PM2_MODERATE) and has been seen in FH patients meeting clinical criteria, including patients where secondary causes of high cholesterol were excluded (PS4_SUPPORTING; PP4_SUPPORTING; PMID: 33668494). Based on the evidence listed above, we have classified this variant as Pathogenic. -
