MYH7 (HGNC:7577): (myosin heavy chain 7) Muscle myosin is a hexameric protein containing 2 heavy chain subunits, 2 alkali light chain subunits, and 2 regulatory light chain subunits. This gene encodes the beta (or slow) heavy chain subunit of cardiac myosin. It is expressed predominantly in normal human ventricle. It is also expressed in skeletal muscle tissues rich in slow-twitch type I muscle fibers. Changes in the relative abundance of this protein and the alpha (or fast) heavy subunit of cardiac myosin correlate with the contractile velocity of cardiac muscle. Its expression is also altered during thyroid hormone depletion and hemodynamic overloading. Mutations in this gene are associated with familial hypertrophic cardiomyopathy, myosin storage myopathy, dilated cardiomyopathy, and Laing distal myopathy. [provided by RefSeq, May 2022]
MHRT (HGNC:51291): (myosin heavy chain associated RNA transcript) This gene encodes a spliced long non-coding RNA that may act as a cardioprotective agent in the heart. Based on a study of a similar gene in mouse, the encoded transcript may regulate chromatin remodeling by acting as a decoy to the BRG1 chromatin repressor complex thus preventing it from binding to its genomic targets. Variant classified as Uncertain Significance - Favor Pathogenic. The p.Ser1463Le u variant in MYH7 has not been previously reported in individuals with cardiomyo pathy or in large population studies. Serine (Ser) at position 1463 is highly co nserved in mammals, and the change to leucine (Leu) was predicted to be pathogen ic using a computational tool clinically validated by our laboratory. This tool' s pathogenic prediction is estimated to be correct 94% of the time (Jordan 2011) . In summary, while there is some suspicion for a pathogenic role, the clinical significance of the p.Ser1463Leu variant is uncertain. -

The p.S1463L variant (also known as c.4388C>T), located in coding exon 30 of the MYH7 gene, results from a C to T substitution at nucleotide position 4388. The serine at codon 1463 is replaced by leucine, an amino acid with dissimilar properties. This variant was reported in individual(s) in hypertrophic cardiomyopathy (HCM) and sudden unexplained death cohorts (Guo L et al. JAMA Cardiol, 2021 Sep;6:1013-1022; Harper AR et al. Nat Genet, 2021 Feb;53:135-142). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
