Variant summary

Our verdict is Uncertain significance. Variant got 3 ACMG points: 7P and 4B. PM1PM5PP3PP5_ModerateBS2

The NM_000458.4(HNF1B):c.853G>A(p.Gly285Ser) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000992 in 1,410,962 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G285D) has been classified as Likely pathogenic.

Genes affected
HNF1B (HGNC:11630): (HNF1 homeobox B) This gene encodes a member of the homeodomain-containing superfamily of transcription factors. The protein binds to DNA as either a homodimer, or a heterodimer with the related protein hepatocyte nuclear factor 1-alpha. The gene has been shown to function in nephron development, and regulates development of the embryonic pancreas. Mutations in this gene result in renal cysts and diabetes syndrome and noninsulin-dependent diabetes mellitus, and expression of this gene is altered in some types of cancer. Multiple transcript variants encoding different isoforms have been found for this gene.[provided by RefSeq, Sep 2009]

ACMG classification

Classification made for transcript

Verdict is Uncertain_significance. Variant got 3 ACMG points.

PM1
In a hotspot region, there are 3 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 3 uncertain in NM_000458.4
PM5
Other missense variant is known to change same aminoacid residue: Variant chr17-37731786-C-T is described in ClinVar as [Conflicting_classifications_of_pathogenicity]. Clinvar id is 635636.We mark this variant Likely_pathogenic, oryginal submissions are: {Uncertain_significance=1, Pathogenic=1}.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.835
PP5
Variant 17-37731787-C-T is Pathogenic according to our data. Variant chr17-37731787-C-T is described in ClinVar as [Likely_pathogenic]. Clinvar id is 562392.Status of the report is criteria_provided_single_submitter, 1 stars. Variant chr17-37731787-C-T is described in Lovd as [Likely_pathogenic].
