This missense change has been observed in individual(s) with clinical features of hypertrophic cardiomyopathy (PMID: 25351510, 29386531). ClinVar contains an entry for this variant (Variation ID: 465907). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in one individual affected with dilated cardiomyopathy (PMID: 29386531) and in one individual affected with hypertrophic cardiomyopathy (PMID: 25351510). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.I228V variant (also known as c.682A>G), located in coding exon 5 of the DSP gene, results from an A to G substitution at nucleotide position 682. The isoleucine at codon 228 is replaced by valine, an amino acid with highly similar properties. This alteration has been reported in a hypertrophic cardiomyopathy (HCM) cohort; however, clinical details were limited (Lopes LR et al. Heart, 2015 Feb;101:294-301). This alteration was also reported in a dilated cardiomyopathy (DCM) cohort (Tobita T et al. Sci Rep, 2018 Jan;8:1998).This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
