The variant received -2 classification points (ACMG Germline Pathogenicity v2019). PVS1_ModerateBS2

The NM_001406810.1(EFNA4):c.1A>G (p.Met1?) variant causes a start lost change involving the alteration of a non-conserved nucleotide. Note: NM_001406810.1 is not a MANE Select or MANE Plus Clinical transcript for EFNA4; the reported annotation may differ from that of the MANE-designated reference transcript for this gene. The variant disrupts the start codon, preventing normal translation initiation. The variant allele was found at a cumulative frequency of 0.0000291 (AC=47) in the gnomAD database across 1,613,898 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.00032. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. Variant has been reported in ClinVar as Uncertain Significance (★).

Genes affected
EFNA4 (HGNC:3224): (ephrin A4) This gene encodes a member of the ephrin (EPH) family. The ephrins and EPH-related receptors comprise the largest subfamily of receptor protein-tyrosine kinases and have been implicated in mediating developmental events, especially in the nervous system and in erythropoiesis. Based on their structures and sequence relationships, ephrins are divided into the ephrin-A (EFNA) class, which are anchored to the membrane by a glycosylphosphatidylinositol linkage, and the ephrin-B (EFNB) class, which are transmembrane proteins. This gene encodes an EFNA class ephrin that has been implicated in proliferation and metastasis of several types of cancers. [provided by RefSeq, May 2022]

Classification according to ACMG Germline Pathogenicity v2019

Classification was made for transcript:

Our verdict: Likely_benign. The variant received -2 points.

PVS1Null variant in LOF gene
Start-lost with downstream start: >10% CDS truncated — moderate (PVS1 downgraded to PM); Start codon lost (exon 3 of 5); downstream rescue start codon present, but it truncates more than 10% of the CDS.
BS2Observed in healthy adult
Pure AD gene with cumulative gnomAD AC ≥5 — observed in healthy adults (BS2); GnomAD homozygous count = 0.; Cumulative gnomAD AC = 47 (pure AD gene, not AR).
